Reviewer:Harohalli Shashidhar, MD (University of Kentucky College of Medicine)
Description:This book describes the pathophysiology, diagnosis, and management of gastrointestinal and liver diseases in children. It describes gastrointestinal function, covers the approach to common clinical gastrointestinal problems, and discusses specific diseases. This third edition successfully incorporates updated information in this rapidly evolving field since the last edition of seven years ago. Changes in the format of this edition include bigger and better text format and addition of color plates of endoscopy and pathology specimens.
Purpose:The preface states that "the goal of the textbook is to incorporate the rapid changes in medical knowledge into a framework that is useful to those providing care in children." This book successfully meets the stated objectives, providing much needed information to both primary and specialist practitioners. This is especially important in the field of pediatric gastroenterology where information is rapidly accumulating and sources of accurate and appropriate information are not yet widely available in the general medical literature.
Audience:The book addresses the needs of practitioners across the spectrum from primary care providers to pediatric gastrointestinal specialists. The approach to common clinical problems provides a useful reference for trainees including medical students/residents. The addition of the section on biologic aspects of gastrointestinal function makes it optimal for a "cover-to-cover" comprehensive read as a basic textbook for fellowship trainees. The expandedlist of authors in this third edition, drawn from both the American and European continents, includes recognized experts in the field of clinical pediatric gastroenterology.
Features:The book offers a comprehensive discussion of anatomic and physiologic aspects of the gastrointestinal tract as it applies to clinical problems and details the approach to common clinical gastrointestinal problems in children. Chapters on common specific illnesses follow, ending with section on practical aspects of nutritional management and a very welcome addition describing pediatric endoscopy techniques. The section on biologic aspects retains clinical relevance across the chapters. The approach to common clinical problems is liberally illustrated with tables, algorithms, and diagrams that lend themselves to power reading and scanning. This edition employs a different format and bigger fonts that is attractive. The addition of color plates at the beginning of endoscopic and histopathologic specimens adds to the overall value. The section on foreign body ingestions details certain nonstandard improvised endoscopic techniques, which would have been more appropriate in a specialized book on pediatric endoscopy.
Assessment:This updated edition retains all the qualities of previous editions that earned it a wide following and recognition. It successfully incorporates several additions enhancing the value. It delivers essential and comprehensive pediatric gastroenterology information to practitioners and trainees. The field of pediatric gastroenterology is not yet cluttered with textbooks and, as such, every book is welcome. However, other books in the field serve some specific needs better than others. For example the multieditor, multivolume pediatric gastrointestinal disease textbook, Pediatric Gastrointestinal Disease: Pathophysiology, Diagnosis, Management, 4th edition, Walker et al. (B.C. Decker, 2004) is more a reference serving the specific needs of specialists and researchers in the area. The multiauthor Textbook of Pediatric Gastroenterology and Nutrition, Guandalini (informa Healthcare/Taylor & Francis, 2004) , while focusing well on specific diseases, lacks the approach to common clinical problems.
