This is an update of the first edition that was published in 1978. This second edition clarifies the pathophysiology of hypertensive diseases in pregnancy. The editors attempt to provide the most recent information on this research area, and give didactic advice for the care of pregnant women with hypertension. This is a multiauthored text, reflecting the enormous progress in the field. In my opinion, this books meets the editors' objectives. It is written for practicing physicians, maternal-fetal medicine specialists, and obstetric investigators in the field of hypertension. All contributors participating in this edition are credible authorities in their field. The pathophysiology of hypertensive diseases in pregnancy is covered. Etiologic aspects, prevention, and management of preeclampsia are particularly well developed and help the reader understand the new concepts of the pathophysiological aspects of preeclampsia. This book is useful for investigators and physicians in the field of hypertension in pregnancy. It is a well-documented book and each chapter is written by an expert in that field. It replaces entirely the first edition since progress in the knowledge of hypertensive disease in pregnancy has been enormous.

"Preeclampsia remains a lethal enigma of pregnancy. Encompassing more than just hypertension, it can include abnormalities of platelet behavior and clotting mechanisms and of endothelial, hepatic, and renal function. The first edition of Leon Chesley's Hypertensive Disorders in Pregnancy (1978) was a scholarly monograph, written by a scientist with a passionate interest in the condition. Since then, the number of maternal deaths from preeclampsia and eclampsia per million pregnancies has been halved in developed countries. Nevertheless, preeclampsia, together with eclampsia, remains one of the two most frequently cited causes of maternal death in the West.
The second edition is a multiauthored book. The three editors are researchers who have made a particular effort to achieve a degree of homogeneity in the presentation. The contents are divided into six main sections, each overseen by one of the three editors. Each chapter is also coauthored by one of the editors, which has resulted in coherence in the writing among chapters. This second edition of a pioneering, but now rare and out-of-print work on hypertension in pregnancy employs several chapter authors (19 in all) to approach the topics at hand, each chapter being coauthored by one of the three editors. The eponymous doctor's participation in this update of his work has been minimal; indeed, only one chapter of his original text, consisting of an historical overview, has been retained in the present volume. However, an editorial introduction to each chapter compares it to the coverage of the relevant material in the first edition. Among the topics addressed are cardiovascular adaptation to normal pregnancy, and the clinical spectrum, pathology, pathophysiology, etiology, prevention, and management of preeclampsia.
